This office’s standard recordkeeping forms are not required for single games. If an organization designs its own forms, refer to the Pull Tab chapter in the administrative rules and the forms listed below for guidance.

Club Special, Seal Board, Tip Board and Punchboard
The maximum price per chance is $2. A prize board, club special, punchboard, sealboard, and tip board are conducted as a single game which may have a cash or merchandise prize and one or more seal prizes. The value of a seal prize or last sale prize may exceed the value of the top tier prize.

Calcutta
A Calcutta is a sporting event in which players wager at an action on the competitors. The conduct of a Calcutta is the auction process. An auction pool is comprised of the wagers paid by players who offered the highest bids on the competitors. The winning competitor may be one competitor, a team of competitors, or ranked competitors. The payout of the cash prize to a winning player is based on a predetermined percentage of the auction pool, which may not exceed ninety percent. Only cash prizes may be awarded.

- The Calcutta board must be purchased from a licensed distributor and contain a North Dakota gaming stamp.
- Calcutta Summary (SFN 18870)
- This form is used to summarize the Calcutta activity for reporting on the tax return.

Prize Board
A prize board is a board used with pull tabs to award cash or merchandise prizes. Coins of various values may be affixed to the board and, under each coin, a cash prize value preprinted on the board. A board must contain numbered lines and a seal covering a winning number.

- This form is used to account for the activity of one prize board at a site.
Prize Board Dispensing Devices
A prize board dispensing device means a board used with pull tabs dispensed from a device to award cash or merchandise prizes. The maximum price per chance is $2. The value of a seal prize or a column sold out indicator may exceed the value of the top tier prize.
- Dispensing Devices – Access Log (SFN 18113)
- This form is used to record the access into a device by any person.
- Credit Redemption Register (SFN 18127)
- This form is used to record the payment to players for pull tabs not properly dispensed by the device.
- Prize Board Dispensing Device Interim Period Site Summary (SFN 58513)
- This form is used whenever currency is withdrawn from the device, pull tabs are bought back from the bar, or a new prize board is put into play.
- Prize Board Dispensing Device Summary (SFN 58514)
- This form is used to summarize a prize board using the information on the applicable Prize Board Dispensing Device Interim Period Site Summary.
- Prize Board Dispensing Device Daily Employee Report (SFN 58515)
- This form is used by an organization employee who is redeeming winning pull tabs for the prize board dispensing device.
