Manufacturers and Distributors of charitable gaming equipment and supplies must be licensed by the Office of Attorney General.
Manufacturers
To become a licensed manufacturer of charitable gaming equipment and supplies, complete and submit the application form along with payment of applicable fees and any required documents. The fee schedule is on the application form:
- Application for License to Manufacture Gaming Equipment and Supplies (SFN 14995); and
- A Request for Record Check (SFN 50424); must be completed for each of the manufacturer’s partners if a partnership, each stockholder owning ten percent or more of the outstanding voting common stock of a corporation, including the corporation’s parent or subsidiary corporation, if any.
- A manufacturer of Electronic Pull Tab Devices must submit a License Bond for Manufacturer of Electronic Pull Tab Devices (SFN 61499) and furnish a $50,000 bond to the Attorney General.
Distributors
To become a licensed distributor of charitable gaming equipment and supplies, complete and submit the application form along with the $1,500 fee and any required documentation:
- Application for License to Distribute Gaming Equipment and/or Supplies (SFN 16021); and
- A Licensing Request for Record Check (SFN 50424) must be completed by each owner, partner, corporate officer, and member or shareholder of 10% or more and each sales representative.
Processing Time
It takes approximately four to six weeks to process the record checks and the application form.
